Early Childhood Screening for Children 3-1/2 to 5 years old

Screening is REQUIRED for entrance to public school kindergarten. To schedule an Early Childhood Screening appointment please click the following link  http://springlakepark.registryinsight.com  Select "Available Classes" from the header, scroll down to "Screening-Early Childhood".
 
Please print and complete all of the following forms for your child's Early Childhood Screening appointment. If you are unable to bring these forms with you to your appointment, you will need to arrive at least 15 minutes BEFORE your appointment to give yourself adequate time to complete the forms.

The years from birth to the start of kindergarten are an important time of rapid learning and growth. Early Childhood Screening is a FREE, quick and simple check of your child`s development. It can help identify, at an early stage, possible health or learning concerns, so that children can get help even before starting kindergarten. Minnesota law requires that your child be screened before enrollment in public school kindergarten. The service is provided at no cost to families and the results of each portion are discussed with the parent or guardian.
  • Earlier is better! The best age to screen is just before the child`s fourth birthday, between 3 1/2 and 4 years of age
  • Screening can help detect possible health or learning concerns so that children can get help before kindergarten
  • Children enrolled in Head Start will be screened there and records are forwarded to their school district
  • Results of each portion are discussed with the parent or guardian

Screening takes an early look at a child's development and is conducted by qualified professionals to help identify specific needs a child may have in the areas of:

  • Vision and Hearing
  • Height and Weight
  • Cognitive and Social Development
  • Fine and Gross Motor Skills
  • Immunizations and Health History
Results of each portion are discussed with the parent or guardian.

District 16's Early Childhood Screening program is located in Woodcrest Elementary School, 880 Osborne Road NE, Fridley, MN 55432; enter through the school's main entrance off of Van Buren Street.

You may also call the Early Childhood office at 763-785-5560 to schedule an appointment for your child. Appointments typically take 45-60 minutes. If possible, please do not bring other children to screening. We have found that siblings distract the child being screened and the process takes longer.
