Registration Information
Registration Forms
Fall registration packets will be available on the SLP website and in the Activities Office by Aug. 2. The fall registration/orientation night is scheduled for Monday, August 2nd at 6:30 in the Fine Arts Center. The Activities office will be open at 6:00pm for registrations on this night. You may also register online or download & print the registration packet and have each form signed by both the athlete and a parent/guardian then turned in to the SLPHS Activities Office with the appropriate sports fee. Please be sure the athlete's sports physical is current (sports physicals are good for 3 years).
