Transition Ages 18-21 Programming
 
“Transition Skills Increase the Opportunity for a Successful Future”
 
The Spring Lake Park School District 16 offers a transition program for students who are 18 to 21 years of age. The Life Skills Transition Program is located in the community offering experiences in all areas of transition. These experiences are based on individual student needs, skill level and IEP goals.
 
Transition Vision Statement
Young adults with disabilities will develop the necessary skills to prepare themselves for successful future.
 
Transition Mission Statement
To connect young adults with disabilities and their families to community resources, empowering them and strengthening their advocacy skills in order to navigate their life plan.
 
Student Profile
  • Special Education students who are between the ages of 18 – 21 who need additional training following high school to prepare for a successful future are eligible to be enrolled in the transition program.
  • All students must have an Individual Education Plan (IEP).
  • A student’s IEP team must make a recommendation for enrollment prior to the end of the senior year with the approval of the Director of Special Education. Or
  • A Student who has dropped out of high school, previously received special education services and is between the ages of 18 – 21. This student may qualify to re-enroll in school through the transition program with a goal of gaining needed skills for a successful future.
What is the Life Skills Transition Program?
  • A community based program for young adults 18 to 21 years of age.
  • A program which provides group and individual community activities.
  • A program which provides opportunity to gain employment skills.
  • A program which provides the opportunity to learn functional skills needed for everyday living.
  • A program which offers leisure activities, individual and group.
  • A program where young adults develop independent living skills.
  • A program which offers individualized learning activities based on student needs.
  • A program which offers young adults Work-based Learning training opportunities in the community.

Northeast Metro Intermediate District 916 Career & Technical Center | Website
The Career & Technical Center delivers relevant, quality technical education by providing career exploration and preparation for employment and continuing education in cooperation with member districts, Century College and employers. The college campus location provides state of the art facilities that enhance the practical skill training with integrated related academics. High school credit and possible college credit can be earned.
 
PAES Lab (Practical Assessment Exploration System)
The PAES lab provides students with general vocational skills and work experience in most careers. The lab includes assessment, work experience and developement of work based behaviors. District 16 was the first district in the State of Minnesota to purchase and implement use of the lab with transition age students. District 16 is a model PAES program and many school districts have visited our program as they consider using this system in their districts.

PAES Lab Teacher Comments
The PAES (Practical Assessment Exploration System) Lab gives students the opportunity to try out 265 different hands-on jobs.  It is an evaluation tool that assists in finding students' strengths and weaknesses in five employment areas: Business/Marketing, Consumer/Service, Construction/Industrial, Processing/Production, and Computer/Technology.  The PAES Lab helps students develop foundational/basic work skills and behaviors.
 
Project SEARCH
The Project SEARCH High School Transition Program is a unique, business-led, one-year, school-to-work program that takes place entirely at the workplace. Total workplace immersion facilitates a seamless combination of classroom instruction, career exploration, and relevant job-skills training through strategically designed internships. District 16 works in collaboration with Centennial Public Schools in providing this service to our students.

CTIC (Community Transition Interagency Committee) is a group of students, parents school staff, public and private personnel, and employers who come together to discuss ways to improve the lives of young adults with disabilities (ages 14- 21) transition from school to a successful future.
